The Department of Health and Aged Care has updated forms for Hearing Services Program (HSP) providers upgrading to digital format and improving accessibility, usability, clarity and readability.
The department said all HSP forms could now be completed digitally, and providers must use the updated forms from 1 July 2025.
It said the updated forms were the final piece of the program’s web transition which involved a new website at health.gov.au/hear as well as an update of content and resources.
“Improvements have been made to accessibility and usability, and you can complete all forms digitally,” the department said. “The forms have been updated to improve readability and give greater clarity for clients completing the forms.
“There is space at the top of the forms for providers to add branding. Providers can adapt the design of the forms for their purposes however they cannot alter the text.
“Please ensure you update your systems to use the new forms from 1 July 2025. You may use the updated forms sooner if this suits your business.”
The department said no new forms had been created.
About 29,500 HSP vouchers issued a month
Meanwhile new data shows that 29,527 HSP vouchers are issued a month.
Almost half a million clients (489,534) were fitted with 220,438 devices in the six months from July to December 2024, the new HSP statistics show.
During this period 68,276 new HSP vouchers and 108,887 return vouchers were issued.
